It's about voting, it's not important if it's not the original theatrical poster, especially if it's crap, ugly or full of text. (i.e the quality isn't as good as more recent physical release or theatrical re-release posters).

No, the primary one is only selected by moderators, it should be a title-only poster of high quality, it will always be shown as the "top poster" regardless of how many votes it has. Mostly comes in handy when voting the title-only high quality one doesn't help to bring it on top and a poster with lots of text or poor quality will be the "top one".
