I rewatched this recently and early in the episode when old Picard was hanging with Geordi in the vineyard, he asks "Why are they laughing at me" about the visions he is having of the freakshow Q jury. But what we see is them angrily shouting, stomping and gesticulating. They sure didn't look to be laughing in any sense of the word. Old Picard really was senile.

How could it have been? YOU are the Nexus!
But of course, once you introduce something like that, you can never be sure of anything any more. Same with the Moriarty episodes where some of them - especially Barclay - wonder aloud about whether they might be holograms too. And Barclay getting no response when he says "computer, end program" proves nothing. Hologram characters aren't supposed to be able to do that! So if he was/we are a holographic simulation, the only way we'd find out is if there was a malfunction similar to what happened in those episodes.
But there might be "side" evidence. In a recent Big Bang Theory episode, Sheldon mentions that Elon Musk has theorized that our reality could be some kind of holographic game being played by aliens. Leonard asks "So some alien kid paid extra for the glasses and asthma upgrade for me?" Sheldon responds "He said it might be a game, he didn't say it was a GOOD game." But it seems more likely to be evidence that it's not a game at all.

I'm not the Nexus of Star Trek I took the name from the Nexus 6 of Blade Runner.
